As an alternate ending to season 2, Sam kills Jake, proving he is the 'chosen one' to lead Azazel's army.
In an effort to save his brother's life, put into danger now that Azazel believes the only way to turn Sam to his side is to kill the one thing 'holding him back' his brother, Sam makes a deal with the demon. Him, in return for letting Dean live. The consequences of this deal being that Sam will forget everything about his former life. Including Dean.
Azazel makes good on his promise, keeping Dean alive as a 'pet'. Dean enduring the torture and humiliation of being a slave to the sadistic demon only because he hopes by doing so he'll be able to eventually reach Sam somehow. But for nearly a year, he does not even see Sam once, and begins to loose hope.
One day Dean is brought into a banquet, along with a few other human slaves, to be used as entertainment. It is the first time he sees Sam, and he is shocked how much his brother has changed. Sitting at the head of the table with Azazel and a few other high ranking demons, celebrating now that the war has nearly been won. There is absolutely no recognition in Sam's eyes when he looks at him.
Dean decides he no longer wishes to live, even hell would be better than this, and decides to kill himself. But he's not prepared instead of being taken back to his cell after the party, or to Azazel's quarters, he's taken to Sam's room. As a 'present' to his brother.
Dean wants to hope this is the chance he's been waiting for, but how can he hope to help his brother at least remember him when Sam sees him as nothing more than an amusing plaything? Especially when he can't even speak to the younger man because Azazel had long since rendered him mute.

I've got a few ideas for this story if anyone is willing to pick it up where it left off. Dean has managed to escape from the hospital and now Sam and his demons are looking for him. Either Dean finds some human rebels just as Sam and his demons find him, or maybe he is rescued from some human rebels. But the rebels manage to exorcise the demons, and perhaps Sam is even injured during the conflict, and the humans take Dean with them back to their base. After confirming he is human, of course, but they still don't exactly trust him. That is, until Bobby sees Dean, recognizes him, and vouches for him.
While Dean is with the humans he heals up some more and manages to tell Bobby a little about what has happened to him, by writing it down and Bobby helps him learn a bit of sign language as well. Dean reluctantly admits Sam's apparent obsession with him, but that even though Dean doesn't think Sam is possessed his brother doesn't know who he is.
The humans come up with a rather suicidal plan of getting rid of Sam, since Sam is the demon's main weapon/general, they want him killed. They decide to use Sam's obvious obsession with Dean to their advantage, by sending Dean back to kill his brother. Of course Bobby is against it, and so is Dean, but they finally convince him that Sam is already as good as a demon and the only way they'll win this war is if Sam dies, but they just can't get anyone in close enough to do the job. Dean finally agrees to do it.
Dean allows himself to be recaptured by the demons, and does his best to get on Sam's good side so he'll have a chance to kill him. The only problem is the longer he is with Sam the more he comes to believe that maybe his brother is still in there somewhere, and he can't kill his brother if theres a chance to bring him back.
